Please make sure to have your current insurance card with you for each of your appointments. Verify with your insurance company that Apple-a-Day/Dr. Webb is in your insurance network. If you have a new policy and have not yet been given your new card, please be able to provide us with the insurance company, claims address, member ID, group ID, policy holder demographics, and copay amounts.
